New coding guidelines and documentation requirements for ambulatory Evaluation and Management (E/M) services take effect January 1, 2021. They are the most radical change to coding and documentation requirements in over 20 years but were developed to reduce clinical documentation burden and give health care clinicians more time to spend with patients.
